Delayed lows after sport

Why a low can arrive hours after exercise, often overnight: muscles restocking, insulin working harder, and which sessions leave the longest tail.

The session went fine. Hours later, or in the middle of the night, your glucose slides anyway. That is a delayed low, and it catches people out precisely because nothing appeared to go wrong at the time. Your muscles are refilling their fuel stores, and they are unusually good at taking glucose in while they do it.

  • Muscles restock the fuel they used, pulling glucose out of your blood for hours afterwards, commonly into the night.

Why it happens

What your muscles are doing for the rest of the day

Muscles keep a local store of fuel, and exercise spends it. Once you stop, they start refilling, and the raw material is glucose taken out of your blood. That restocking is not quick: depending on how hard and how long you went, it can run for many hours, and it does not politely stop because you have gone to bed.

On top of that, activity leaves your body more responsive to insulin for a while, often well into the next day. That is a genuinely good effect and one of the real health benefits of exercise. It also means the insulin you already take is doing more work than usual during that window, which is the second half of why the low arrives late.

Put together: your muscles are actively pulling glucose out, and your insulin is working harder than normal, both at a time when you are probably asleep and not checking. That is the whole mechanism, and knowing it is most of the defence.

When it lands

Which sessions produce the longest tail

Roughly, the more fuel you spent and the less used to it you are, the longer the tail. A long steady effort empties the stores more thoroughly than a short one. A session harder than your usual leaves a bigger deficit. And a first session back after a break is the classic setup, because your muscles are both emptier and more responsive than they will be a month from now.

Timing matters as much as size. An afternoon or evening session puts the restocking window right across your night, which is why evening sport has a reputation for overnight lows that morning sport does not. The same session done at eleven in the morning often shows up as an afternoon dip you notice and handle easily.

Alcohol afterwards compounds it, because your liver deals with alcohol before it gets round to releasing stored glucose, taking away the backstop you would otherwise have overnight. A match followed by a few drinks is a genuinely different proposition from the match alone.

Catching it, and what you can do about it yourself

The part you own is carbohydrate and information. Eating something after a session, particularly one with a mix of quick and slower carbohydrate, gives the restocking something to work with other than your bloodstream. Checking before bed on an active day, and again if you wake, turns an invisible fall into one you can respond to. If you use a sensor, an overnight low alarm is worth having switched on for those nights specifically.

The part your team owns is insulin. There are recognised ways of handling background insulin around activity, and they are genuinely useful, but they are decisions to make with your team rather than adjustments to try alone, because getting them wrong runs in the direction of a bigger low rather than a smaller one.

Then treat the pattern as learnable. Note the session, roughly how hard, what time, and what your glucose did overnight. After a handful you will know whether your particular tail is three hours or ten, and that is a far more useful thing to bring to an appointment than “I go low sometimes after sport”.

An unfamiliar or unusually long session leaves the biggest deficit and the longest tail. Evening activity puts the window across your sleep, which is where a delayed low does the most damage and gets noticed least. Alcohol afterwards removes the liver’s usual backstop. Saturday afternoon football went well and your glucose behaved all evening. At two in the morning you wake up low, or your alarm goes off. Nothing failed during the game; the restocking simply arrived on its own schedule.

The first session back

Three weeks off, then a single ordinary gym session, and that night is the lowest you have been in months. Emptier stores and a body less used to the work make the first one back the one to watch most carefully.

The match and the pub

The game itself was fine and so was dinner. Two drinks afterwards, and overnight the fall keeps going, because your liver is busy with the alcohol instead of topping you up the way it normally would.
