Why your ratios differ across the day

Why the same carbs can need different insulin at breakfast than at dinner, and what shifts across the day.

If the same amount of carbs seems to need a different amount of insulin at breakfast than at dinner, you’re not imagining it. Your body’s sensitivity to insulin shifts across the day, so one ratio doesn’t fit every meal.

  • A carb ratio (the grams of carbohydrate one unit of insulin covers) often needs to be different at different meals, and that’s normal, not a mistake you made.

What this means when you’re actually eating

The practical version: a slice of toast at 7am and the same slice at 3pm can behave differently, even though the carbs haven’t changed. That’s why insulin regimens and pumps often have more than one carb ratio set up, one for morning, one for the middle of the day, one for evening. If yours only has a single number, or you’re doing it in your head from one figure, it’s worth knowing that’s a simplification, not a fixed truth.

You don’t need to work any of this out yourself. What helps is noticing a pattern: if one meal reliably runs high afterwards while others sit fine, that’s information for your team, not a sign you’re getting it wrong. Ratios are estimates that get refined over time, including after long gaps, and refining them is the ordinary work of living with type 1.

The mechanism

Why the same carbs need different insulin

Your body makes hormones on a daily cycle: cortisol, growth hormone and others. Several of them push glucose up and make insulin work less well. They tend to peak in the early morning, which is why a lot of people wake with rising glucose and find breakfast the hardest meal to cover. This is sometimes called the dawn effect. By the afternoon those hormones have eased off, so the same food may need less insulin per gram of carb.

On top of that, what you did earlier in the day changes things. Activity, a previous meal still being digested, stress, illness and sleep all shift how sensitive you are hour to hour. So a ratio is a best estimate for a typical version of that time of day, not a fixed exchange rate.

Movement

How activity pulls sensitivity in both directions

Exercise is one of the biggest reasons your sensitivity moves, and it doesn’t move one way. Gentle or steady activity, a brisk walk, an easy cycle, a swim at a comfortable pace, usually makes insulin work better for hours afterwards, so you can need less insulin and be more prone to going low. Intense or competitive effort, sprint intervals, heavy lifting, a match where the adrenaline is up, often does the opposite in the moment: stress hormones push glucose higher, and you may sit high for a while.

The part that catches people out is timing. After harder or longer activity, a low can arrive during the session or straight after, and also hours later, including overnight, because your muscles keep pulling glucose in to refill their stores. So the meal after exercise, and the ratio you’d normally use for it, can behave very differently from usual. Anything that would actually change a dose around exercise is a conversation for your team.

None of these are fixed for everyone. Two people can have opposite morning patterns, and your own can change over weeks or years. That’s why there’s no universal ratio: there’s a pattern for you, at this point, that your team helps you read. When several of these stack up (a bad night, a cold coming on, a hard session the day before), expect the usual numbers to be less reliable.

The same toast, morning and afternoon

You have toast at breakfast and go high afterwards, then the same toast mid-afternoon sits steady. That’s the dawn effect: morning hormones are making insulin work less well, so breakfast often needs proportionally more cover than later meals.

The evening after a hard session

You’ve played a competitive match and your glucose ran high during it from the adrenaline. Hours later, at dinner or overnight, you drift low as your muscles restock. The meal you’d normally judge easily behaves differently that evening.

A steady walk before lunch

You’ve walked 30 minutes into town before lunch. Insulin is working better than usual for the next few hours, so a lunch that’s normally fine leaves you lower than expected. Steady movement raises sensitivity, sometimes well into the afternoon.

Worth paying attention to

A single meal that reliably runs high or low afterwards, day after day, when the food and dose are much the same: that pattern is what your team wants to hear about, not a one-off reading.
Check your glucose before eating and again about two to three hours after, when fast insulin has mostly finished, to see whether a meal actually landed where you expected.
After harder or longer activity, keep an eye on your levels into the evening and overnight, because lows can turn up hours later.
A treatment for a low glucose (a hypo) close to hand is routine, not an emergency: fast sugar, then recheck.
